اثربخشی طرحواره درمانی بر اضطراب و افسردگی زنان مراجعه‌کننده به متخصص توانبخشی در سال 1394

چکیده

مقدمه: اختلالات‌‌ افسردگی و اضطرابی از شایع‌ترین اختلالات روانپزشکی در جمعیت عمومی هستند که به صورت بحرانی افزایش یافته­اند. لذا هدف پژوهش حاضر، بررسی اثربخشی طرحواره درمانی بر اضطراب و افسردگی زنان مراجعه­کننده به متخصص توانبخشی بود.

مواد ‌و‌‌ روش‌ها: پژوهش حاضر، یک مطالعه نیمه تجربی از نوع پیش‌آزمون و پس‌آزمون با گروه کنترل و جامعه آماری شامل  بیماران زن مراجعه­کننده به متخصص توانبخشی همدان بود. نمونه­ها شامل 36 نفر بودند که از این تعداد 18 بیمار در گروه آزمایش و 18 بیمار در گروه کنترل قرار گرفتند. نمونه­ها به روش نمونه‌گیری تصادفی مبتنی بر ملاک ورود انتخاب شدند. ارزیابی روانی شامل مصاحبه بالینی بر طبق DSM-5،‌ پرسشنامه  اضطراب و افسردگی بک بود. طرحواره درمانی طی 8 جلسه (هفته‌ای 2 جلسه) 2 ساعته در مورد گروه آزمایش انجام شد. پیش­آزمون، پس­آزمون و پیگیری 1ماهه بعد از دوره آموزش در مورد هر دو گروه انجام شد و داده­ها به کمک آماره­های توصیفی و آزمون تحلیل واریانس آمیخته تحلیل شدند.

یافته ­ها: نتایج به دست آمده حاکی از تأثیر طرحواره درمانی بر کاهش اضطراب و افسردگی در گروه آزمایش نسبت به گروه کنترل بود (01/0p<). یافته‌ها نشان دادند که شاخص‌های اضطراب و افسردگی پس از مداخله درمانی به شیوه‌ طرحواره درمانی در دوره پیگیری به طور معنی­داری کاهش یافتند (01/0p<).

نتیجهگیری: برنامه طرحواره درمانی می­تواند جزء برنامه­های مداخلاتی در سطوح گروه­های بیماران اختلالات خلقی در مراکز مشاوره و بیمارستان‌های عمومی قرار گیرد و سلامت روان بیماران مبتلا به اختلالات جسمی را ارتقاء دهد.

Introduction: Anxiety-and depression-related disorders are among the most prevalent psychological disorders and increasingly concern in the general population. Hence the aim of this study was to investigate the impact of remedial schema on anxiety and depression among clients referring to rehabilitation specialist.

Materials and Methods: The current research method is semi-experimental of pre- and post-test type with the control group and statistical community including all female patients referred to rehabilitation specialist. Eighty patients in trial group and 18 patients in control group were selected using entrance criterion through random sampling. Psychiatric evaluation included the clinical interview in accordance with DSM-5, scale of Beck anxiety survey (1988), and second edition of Beck depression survey (2000). Remedial schema was prepared during eight sessions of two-hour (two sessions every week) and no training was applied to the control group. For each of the two groups, pre- and post-test scores, and one-month follow up after training were obtained. The data were analyzed using description statistics and a mixed-design analysis of variance model

Results: The obtained results indicated that the schema therapy had a significant anxiety- and depression-relief effects in the trial group compared to the control group (p<0.01). Similar effects were also observed   in the follow-up period (p<0.01).

Conclusion: According to the findings of the present study, , schema therapy can promote mental health status in patients suffering from physical disorders and may be a part of interventional programs for patients with mood disorders in consulting centers and general hospitals.
