چکیده

مقدمه: رضایت زناشویی از جمله سازه‏ هایی است که به آسانی تحت تأثیر روابط بین زوجین قرار می‏گیرد و آمارها حاکی از افزایش طلاق به دلیل نارضایتی زناشویی است. این پژوهش با هدف مقایسه اثربخشی درمان تلفیقی (زوج‏درمانی هیجان‏مدار و واقعیت درمانی) با طرح‏واره درمانی بر رضایت زناشویی در زوجین ناهمساز و متعارض انجام شد.
مواد و روش‌ها: پژوهش از نوع تجربی تک موردی (Single-subject design) با طرح پیش ‏آزمون، پس ‏آزمون و پی‏گیری بود. جامعه آماری در برگیرنده، زوجین متعارض مراجعه‏ کننده به کلنیک مشاوره پارسای بندرترکمن سال 1400 بود که از میان آن‏ها، 6 زوج (زن و شوهر) بر اساس ملاک‏ های ورود به پژوهش، مصاحبه بالینی ساختاریافته و پرسش‏نامه رضایت زناشویی (Olson و همکاران، 2003) و با نمونه ‏گیری هدفمند انتخاب شد. آزمودنی‏های دو گروه، 12 جلسه هفتگی (دو جلسه، 90 دقیقه ‏ای) برنامه درمانی دریافت کردند. برای تجزیه و تحلیل داده ­ها از شاخص ‏های آمار توصیفی، آزمون مجذور کای، تحلیل دیداری نمودار با شاخص تغییر پایا و درصد بهبودی استفاده شد.
یافته‌ها: یافته‌های پژوهش نشان داد هر دو روش درمان تلفیقی و طرح‏واره درمانی تغییرات معنی ‏داری از نظر آماری و بالینی در آماج‏های درمانی ایجاد کرده (05/0>p)، و تأثیرات درمانی در دوره پی‏گیری نیز ادامه داشته است (96/1 ≤RCI)، اما درصد بهبودی در آزمودنی‏های گروه درمان تلفیقی (018/0=p و 70/38=PND) نسبت به آزمودنی‏ های گروه طرح‏واره درمانی (24/0=p و 45/27=PND) بیشتر بوده است.
نتیجه‌گیری: درمان تلفیقی بر افزایش رضایت زناشویی زوجین متعارض و ناهمساز مؤثر بوده و از آن می‌توان در مراکز درمانی جهت ارتقای کارکرد خانوادگی و زناشویی استفاده کرد.

Introduction: Marital satisfaction is one of the variables easily affected by the relationship between couples. Statistics show an increase in divorce due to marital dissatisfaction. Therefore, this study compared the effectiveness of combined therapy with schema therapy on marital satisfaction in heterogeneous and conflicting couples.
Materials and Methods: The research was a single case study with pre-test, post-test, and follow-up design. The statistical population included conflicting couples referred to the Parsa counseling clinic in Bandar-e-Torkmen County in 2021. Among them, six couples (husband and wife) were selected based on the research entry criteria, structured clinical interview, and marital satisfaction questionnaire (Olson et al., 1987) using purposive sampling. The subjects of the two groups of combined therapy and schema therapy received 12 weekly sessions (two sessions, 90 minutes each) separately. Descriptive statistics, Chi-squared test, visual analysis of graph with reliable change index (RCI≥ 1.96), and recovery percentage were used in the results report.
Results: The findings of the research showed that both methods of integrated treatment and schema therapy had made statistically and clinically significant changes in the treatment goals (p<0.05), and the therapeutic effects continued in the follow-up period (RCI≥ 1.96). However, the percentage of recovery in the combined therapy group (PND=38.70 & p=0.018) was higher than in the schema therapy group (PND=27.45 & p=0.024).
Conclusion: Combined therapy was effective in increasing the marital satisfaction of couples; thus, it can be used in medical centers to improve family and marital function.
