مقایسه اثربخشی درمان شناختی رفتاری و پذیرش و تعهد بر پریشانی هیجانی و سبک زندگی مرتبط با چاقی در افراد مبتلا به اضافه وزن با رفتار خوردن آشفته: یک مطالعه نیمه تجربی

چکیده
مقدمه: چاقی و اضافه‌وزن یک مشکل عمده سلامتی است. هدف مطالعه حاضر مقایسه اثربخشی درمان شناختی رفتاری و پذیرش و تعهد بر پریشانی هیجانی و سبک زندگی مرتبط با چاقی در افراد مبتلا به خوردن آشفته بود.
مواد و روش ­ها: این پژوهش نیمه تجربی با طرح پیش­آزمون-پس­آزمون و پیگیری با گروه کنترل بود. جامعه آماری تمامی مراجعه‌کنندگان مبتلا به اضافه‌وزن به دو کلینیک چاقی شهر تهران از دی 1402 تا خرداد 1403 به تعداد 200 نفر بودند. با نمونه‌گیری در دسترس و هدفمند 45 نفر انتخاب و با قرعه‌کشی در سه گروه درمان شناختی رفتاری، تعهد و پذیرش و کنترل (هر گروه 15 نفر) قرار گرفتند. برای گروه‌های آزمایش، درمان شناختی رفتاری و تعهد و پذیرش 8 جلسه اجرا شد و گروه کنترل درمانی دریافت نکرد. ابزار پژوهش پرسشنامه‌های پریشانی هیجانی Simmons و Gaher و سبک زندگی مربوط به چاقی Clark و همکاران بود. برای تجزیه و تحلیل داده­ها از آنالیز واریانس دوطرفه با اندازه‌گیری­های مکرر استفاده شد.
یافته ­ها: میانگین و انحراف معیار نمره سبک زندگی مرتبط با چاقی درگروه ، شناختی رفتاری در پیش‌آزمون 39/6±61/103 و پس­آزمون 65/5±07/75 تفاوت معنی‌دار داشت، هم‌چنین در گروه تعهد و پذیرش به ترتیب 24/5±58/103 و 16/5±27/88 تفاوت معنی‌دار داشت (05/0>p). میزان بهبودی گروه درمان تعهد و پذیرش در نمرات پریشانی هیجانی به طور معنی­داری بیشتر از گروه درمان شناختی رفتاری بود (032/0=p).
نتیجه­ گیری: درمان تعهد و پذیرش مداخله‌ای کارآمدتر در کاهش پریشانی هیجانی در افراد مبتلا به اضافه‌وزن می‌باشد. استفاده از آن در کنار سایر روش‌های درمانی به درمانگران و متخصصان تغذیه توصیه می‌شود.

Introduction: Obesity and overweight are major health problems. The present study aimed to compare the effectiveness of cognitive behavioral therapy and acceptance and commitment on emotional distress and lifestyle related to obesity in overweight individuals with disordered eating.
Materials and Methods: This is a a quasi-experimental study with a pre-test, post-test design, and with a control group and a two-month follow-up. The statistical population consisted of all overweight individuals with disordered eating behaviors who referred to two obesity treatment clinics in Tehran between January 2023 and June 2024 (200 individuals). 45 people were selected using convenient and purposive sampling and randomly assigned to three cognitive behavioral therapy commitment and acceptance, and control groups (15 people in each group). Cognitive-behavioral, commitment, and acceptance therapies were done for 8 sessions in the experimental groups, with no treatment for the control group. Emotional distress questionnaires (Simmons and Gaher) and obesity-related lifestyle questionnaires (Clark et al) were used. Data were analyzed using two-way repeated measures analysis of variance (ANOVA).
Results: The mean lifestyle-related obesity score of the cognitive behavioral group decreased from pre-test (103.61 ± 6.39) to post-test (75.07 ± 5.65); in the commitment and acceptance group, this score decreased from 103.58 ± 5.24 in the pre-test () to 88.27 ± 5.16 in the post-test (). The improvement rate in emotional distress scores in the commitment and acceptance treatment group was significantly greater than the cognitive behavioral therapy group. (P=0.032).
Conclusion: Commitment and acceptance therapy is a more effective intervention in reducing emotional distress in overweight individuals. Its use, alongside other treatment methods, is recommended by therapists and nutritionists.
